During the investigation it was found that a caregiver was not supervising a child appropriately, resulting in the child being injured.

2 out of 2 records reviewed was missing the parents signature.

Based on evidence obtained during the investigation it was found that the person in charge of the operation at the time of the incident did not use good judgement by reentering the building after they were told to evacuate the building by a gas company professional and allowing parents and children to reenter the building without receiving clearance from the gas company professionals to reenter.

It was found during the investigation that at the time of the gas leak the person left in charge of the facility did not follow/know all the steps outlined in their Emergency Preparedness Plan. It was found that the planned evacuation routes for each class were not followed and instead of evacuating to the planned evacuation site stated in their Emergency Preparedness Plan they evacuated to the playground. It was also found that the gates on the playground were locked, and the employees present at the time of the evacuation did not know the location of the keys to unlock the gates therefore they were unable to exit through the playground gates.

The facility did not have 3 of 14 records avialble for review during in the inspection.

It was found during the investigation that the operation staff did not evacuate with the authorization for emergency care documents for each child in care.

During the walk through of a classroom it was it was found the children did not have soap for hand washing and caregiver stated they had not had soap all morning and was using hand sanitizer to wash the children's hands.

2 out of 10 records reviewed did not have verification that the caregivers meet the education requirements.

Per caregiver interviews it was found that a caregiver inappropriately disciplined a child by grabbing the child's shirt and yanking them.

During the investigation, it was determined that the staff was aware of a child receiving medical attention after sustaining an injury while in care, but did not report to Child Care Licensing within two days of becoming notified.

It was found during the inspection loose fill was not being maintained at the required depth around climbing structures and at the end of the slides.
